Participation Risk: I understand and acknowledge that participation in lacrosse practices, training sessions, clinics, camps, strength and conditioning activities, games, tournaments, team events, and other related athletic activities involves inherent physical risks. These risks may include, but are not limited to, sprains, strains, fractures, concussions, head or facial injuries, cuts, bruises, dental injuries, communicable illnesses, heat-related illness, and other serious injuries. In rare circumstances, athletic participation may result in permanent injury, disability, or death. I understand these risks and voluntarily permit the participant to take part in The Woodlands College Park Lacrosse activities. Medical Authorization In the event of injury or illness, I authorize The Woodlands College Park Lacrosse coaches, staff, volunteers, event personnel, facility personnel, athletic trainers, emergency responders, and other appropriate personnel to obtain or provide medical care reasonably considered necessary for the participant. This may include first aid, emergency medical treatment, ambulance transportation, hospital treatment, or other appropriate medical services. I understand that reasonable efforts will be made to contact a parent or legal guardian when appropriate, but emergency treatment may be provided when immediate care is necessary. I accept responsibility for medical expenses incurred on behalf of the participant that are not otherwise covered by insurance. I certify that I have provided accurate information regarding the participant's health, medical conditions, allergies, medications, and emergency contact information. I agree to notify the program if the participant's medical status changes.

In consideration of the participant being permitted to participate in The Woodlands College Park Lacrosse activities, I knowingly and voluntarily accept the risks associated with participation. To the fullest extent permitted by applicable law, I agree to release and hold harmless The Woodlands College Park Lacrosse, its coaches, directors, officers, employees, volunteers, representatives, sponsors, event organizers, participating facilities, property owners, and other affiliated persons or organizations from claims arising from the ordinary risks associated with participation in program activities. I understand that this agreement is not intended to waive rights or protections that cannot legally be waived under applicable law. I further agree that if the participant is a minor, I am entering into this agreement as the participant's parent or legal guardian and have authority to provide this consent.

I understand that photographs, video recordings, livestreams, or other media may be created during practices, games, tournaments, team events, awards ceremonies, camps, clinics, or other program activities. I grant The Woodlands College Park Lacrosse permission to photograph, film, or otherwise record the participant and to use such media for legitimate program purposes, including: - Team website content - Social media - Game and season highlights - Promotional materials - Team communications - Fundraising materials - Community outreach - Historical or archival purposes I understand that such media may be publicly available and that no compensation will be provided for its use. I understand that I may contact the program in writing if I wish to withdraw future consent for promotional use, although previously published materials may not always be removable.

Participation in The Woodlands College Park Lacrosse carries an expectation of appropriate conduct from players, parents, guardians, spectators, coaches, and volunteers. Players are expected to: - Treat teammates, coaches, opponents, officials, and spectators with respect. - Follow instructions from coaches and authorized program personnel. - Attend practices, games, and team activities prepared and on time. - Demonstrate good sportsmanship. - Avoid bullying, hazing, harassment, fighting, discriminatory conduct, or threatening behavior. - Respect facilities, equipment, uniforms, and team property. - Follow applicable team, league, facility, and school rules. - Refrain from conduct that may endanger themselves or others. Parents and guardians are expected to: - Support players and coaches in a constructive manner. - Treat officials, coaches, players, opponents, and other families respectfully. - Refrain from abusive, threatening, disruptive, or inappropriate sideline behavior. - Allow coaches to coach and officials to officiate. - Communicate concerns appropriately and through designated program channels. - Ensure players arrive prepared and are picked up promptly from team activities. The program may take appropriate action when these expectations are violated, including warnings, removal from a practice or event, suspension, or dismissal from the program when circumstances warrant.

By completing this registration, I certify that: - I am the participant's parent or legal guardian, or I am otherwise legally authorized to complete this registration. - The information I have provided is accurate and complete to the best of my knowledge. - I understand the nature of lacrosse and the physical risks associated with participation. - I have disclosed relevant medical information necessary for the participant's safe participation. - I understand that participation may be subject to team rules, league rules, eligibility requirements, facility rules, and coaching decisions. - I understand that registration does not necessarily guarantee playing time, a particular team assignment, position, jersey number, or roster status unless specifically stated by the program. - I agree to communicate changes in contact, medical, or emergency information to the program. - I have reviewed the agreements contained on this registration page.
